Could you include connect method in nginx to give us an alternative to
squid?
Squid can be notorious for its CPU usage.
There may be a Klondike bar in it for you.

Check out Apache Traffic Server, which has a forward-proxy mode and
certainly seems to be a lot more modern in design than squid:
