Any recommendations on hosting providers (US) that offer a fully
managed dedicated server with rails support?

+1 on Rimuhosting. They’re a new zealand company, but their servers
are located in the US.

I also know people that are very happy with layeredtech.com and
bytemark.co.uk.

+1 on Bytemark. They will give you a dedicated linux box with root
access and you can install rails and whatever else you need yourself.
they’re also very helpful and give personal service and helpful advice.

Didn’t he ask for managed dedicated server ?

layeredtech is not managed. I’m not sure if rimu is managed or not.
