Creation of Bookmark

Dear friends,

I want create one button in my app which can be placed on browsers
bookmark toolbar and can be used to bookmark websites, when we click on
it, it will take title and domain name of that page (website) and will
show a form from my app.
After submitting that form it will make changes to my database.

Does anybody know how to do this? Please give me reply.

Try using a URL. The button itself could be composed of javascript.
