There is a mailer extension for sending forms.  You can then edit the 
extension for multi step based on the railscast on the issue (would link 
but on my phone) or use a jquery plugin for multistep form wizards 
(search jquery form wizard and you should find one).

Does anyone have step by step instructions for using 'mailer' and 
'forms' to create  an assessment form that gets sent to a certain email. 
It would be most helpful if this form could be paginated.
