Could the spam be coming from other sources, such as ruby-forum.com or ruby-core@ruby-lang.org? Do you have to have a redmine account to send an email?

They're sending using redmine directly. Because the mail is coming from redmine, and account link exists on redmine. I'm locking their accounts when I found spammer. (lock = disable an account on redmine admin tool)
